Daily Stair Climbing Linked to Longer Life in Large Global Study

For many people, the choice between a staircase and an elevator lasts only a few seconds. One requires a brief burst of effort; the other removes it almost entirely. But a growing body of research suggests that those small decisions, repeated over years, may be associated with differences in health and longevity.

The evidence was brought together in a large systematic review and meta-analysis presented by researchers led by Dr. Sophie Paddock of the University of East Anglia and Norfolk and Norwich University Hospital Foundation Trust. The analysis combined nine studies involving 480,479 people, ranging in age from 35 to 84. About 53% of participants were women, and the studies included both generally healthy people and people with previous cardiovascular disease.

Compared with people who did not climb stairs, those who did were associated with a 24% lower risk of death from any cause and a 39% lower risk of death from cardiovascular disease. Stair climbing was also associated with lower risks of cardiovascular events including heart attack, heart failure and stroke.

The numbers do not mean that a staircase itself guarantees a longer life. The researchers were examining associations in existing studies, not assigning people to climb stairs and then observing whether they lived longer. That distinction has become increasingly important as researchers examine how everyday physical activities may reflect both fitness and underlying health.

Still, the appeal of the finding lies partly in how ordinary the activity is.

Stair climbing requires no membership, special equipment or dedicated exercise space. It can occur while entering a building, returning home, moving between floors at work or choosing a staircase instead of an escalator. Because climbing stairs involves lifting the body's weight repeatedly against gravity, even a short bout can demand substantially more effort than walking on level ground.

That makes stairs an example of how physical activity can be incorporated into ordinary routines rather than reserved for formal exercise sessions.

Paddock said the evidence supported incorporating stair climbing into daily life, while emphasizing that even brief periods of physical activity can have health benefits. She also cautioned that the apparent relationship between greater stair use and greater benefit needed further confirmation.

The findings build on earlier research. A 2021 analysis of 280,423 participants in the UK Biobank found that people reporting more than five flights of stairs climbed at home each day had a lower risk of all-cause mortality than those reporting no stair climbing. The lowest observed risk was among those climbing six to 10 flights a day, with an estimated hazard ratio of 0.91. The researchers calculated that this association corresponded to roughly 44 to 55 additional days of survival.

But that study also illustrates why the story is more complicated than a prescription to count flights.

The UK Biobank researchers found no clear reduction in cardiovascular mortality associated with stair climbing in their analysis. They also warned that the magnitude of the association was small and susceptible to residual confounding, concluding that climbing stairs at home alone was unlikely to provide enough physical activity to lower premature mortality.

Earlier research among 8,874 older men in the Harvard Alumni Health Study similarly found that greater stair climbing was associated with lower all-cause mortality. Men reporting at least 35 floors a week had a hazard ratio of 0.84 compared with those reporting fewer than 10 floors. The researchers, however, found no statistically significant association with cardiovascular mortality.

Together, the studies point toward a consistent association while leaving open the question of causation.

That question has received renewed attention in 2026. A review published in Healthcare examined the growing evidence connecting stair climbing with cardiovascular and overall mortality and warned that observational findings may partly reflect reverse causation. People who are already healthier and fitter may be more capable of climbing stairs, while people beginning to experience an undiagnosed decline in health may avoid them.

The distinction matters because stair climbing is not a passive measure of movement. It can be demanding, particularly for people with limited cardiovascular or physical capacity. The 2026 review therefore called for further research before treating stair climbing as a universally appropriate preventive intervention.

Other recent research has nevertheless added to the picture. A 2026 analysis of physical activity types found that, among the activities examined, climbing stairs was associated with a lower risk of death. Participants in the highest stair-climbing category had a pooled hazard ratio of 0.90 compared with those in the lowest category. The same study found that people engaging in a wider variety of physical activities had lower mortality than those whose activity was less varied.

For public-health researchers, the significance may therefore lie less in stairs as a magic number than in what they represent: opportunities to add movement to a day that might otherwise be dominated by sitting.

More than one in four adults worldwide do not meet recommended levels of physical activity, according to the European Society of Cardiology, making activities that can be incorporated into existing routines potentially useful.

A staircase cannot establish how long an individual will live. Nor does the evidence establish that climbing a particular number of flights each day will prevent disease. But across different populations and study designs, regular stair climbing has repeatedly appeared alongside better health outcomes.

For someone standing at the bottom of a staircase, the decision remains a small one. The research suggests that, repeated often enough, such small decisions may be part of a much larger pattern of daily movement associated with a healthier and potentially longer life.
