Iowa Sets December 2026 Start Date for Medicaid Work Requirement Enforcement

DES MOINES, Iowa — Iowa will begin enforcing Medicaid work requirements on Dec. 1, 2026, becoming one of a small number of states moving ahead of the federal deadline for implementing the new eligibility standard, according to state and federal policy documents released this year.

The Iowa Department of Health and Human Services said certain adults enrolled in the Iowa Health and Wellness Plan, the state’s Medicaid expansion program, will be required to meet monthly “community engagement” requirements beginning Dec. 1. The requirements stem from federal legislation enacted in 2025 that established nationwide Medicaid work standards for certain adult beneficiaries.

Under guidance published by Iowa HHS, affected beneficiaries generally must complete at least 80 hours per month of qualifying activities, including employment, job training, education, or community service, unless they qualify for an exemption. The state said the requirement applies primarily to adults ages 19 through 64 enrolled in the Iowa Health and Wellness Plan, with exemptions for groups including pregnant individuals, certain caregivers, people deemed medically unable to work, and others meeting specified criteria.

“By December 1, 2026, certain Iowa Health and Wellness Plan members must meet monthly requirements to enroll and maintain coverage,” Iowa HHS said in information posted on its website.

The federal Centers for Medicare & Medicaid Services issued an interim final rule on June 1 establishing a national framework for the new work requirement. CMS said states generally must implement the requirement no later than Jan. 1, 2027, although states may adopt earlier timelines. CMS Administrator Dr. Mehmet Oz said the rule was designed to provide states with “clear expectations” for eligibility determinations, exemptions, verification procedures and reporting requirements.

Health policy researchers have identified Iowa as one of the few states planning early implementation. According to tracking reports published by KFF, Nebraska began enforcing federal Medicaid work requirements in May 2026, Montana is scheduled to begin enforcement in July 2026, and Iowa plans to follow in December. Most other states are expected to implement the requirement at the start of 2027.

Supporters of work requirements have argued that linking benefits to employment, education, or community activities promotes workforce participation and economic self-sufficiency. Some health policy advocates, however, have raised concerns that eligible beneficiaries could lose coverage because of administrative barriers or reporting difficulties.

Iowa officials have said outreach efforts will continue throughout 2026 to help beneficiaries determine whether the requirement applies to them and to explain available exemptions. As of Thursday, the Dec. 1, 2026 implementation date remained the state’s published schedule for enforcement.
